Download, fill and sign the Registration Form before coming in. Ensure both you and your child have a working email address.

Originals/Copies of the following documents must be submitted with the Registration Form.

SUPPORTING DOCUMENTS:

Birth Certificate
Passport Photos (2)
SEA Placement Slip
Clinical Assessment (if child was assessed for a Learning Disability),
Medical Report (if child has a medical condition)
Court Order (if one parent has custody)
Parent/Legal Guardian National Identification Card
National Identification Card of persons authorized to collect the child from school
